If you change name, address or telephone number, please let our receptionists know by calling or writing to us as soon as reasonably possible.
You are also able to do this by clicking the ‘Online Services’ on the left of this page, and filling out the ‘Change Personal Details’ form.
You may be required to attend the practice with proof of your change of details, such as a marriage certificate.
If you move outside the practice area, you may be required to find a doctor in your new area. There are instructions on what to do when you move to a new area on the back of your NHS medical card.
